Insurance
I have chosen to be independent of insurance contracts to provide the best, most attentive care. Most private health insurance plans provide some level of out-of-network reimbursement. If you see the terms “POS”, “PPO” or “out-of network” (OON) on your insurance card then you are likely eligible. If you see “HMO”, “Medicare” or “Medicaid” then you are probably do not. Check with your insurance company or HR rep to be sure.
Filing your OON claim is simple: you come to your session and pay for the visit. I will give you a detailed receipt (known as a “superbill”) that has the necessary information on it to file a claim.
Here are the reimbursement codes so you can check with your insurance company your OON benefits:
- Initial Consultation (Evaluation) — CPT code 90791
- Individual Psychotherapy, 45-minutes follow-up — CPT code 90834
- Couples Psychotherapy, 45-minutes follow-up — CPT code 90847
Cancellation Policy
If you do not show up for your scheduled therapy appointment, and you have not notified us at least 24 hours in advance, you will be required to pay the full cost of the session.
